The Community Dietician Service can help you with:
Individual Referrals
Referrals to the dietitian’s service can be made by primary care providers. Referral forms are available on Medtech. Before referring anyone to this service they must have had basic nutrition advice provided by their primary care provider and deemed to benefit from further in depth advice.
Group Education Sessions
Louise would be happy to attend community groups, exercise groups or already established self management groups to run interactive nutrition education sessions.
Health Provider Education
Nutrition education sessions are available for primary care staff who would like to work more effectively with patients with long term conditions. Louise can come to your practice to deliver a fun, interactive, practical hands-on session about food.
Who can access this service?
Priority groups for the service include Maori, Pacific, migrants, refugees and people on a low income. All other referrals can continue to be sent to the hospital dietitian services.
